Notice to Texas Clients & Consumers

In accordance with Texas Health & Safety Code §181.105 (House Bill 4224), the following information is provided to ensure transparency and protect your rights as a client.

Requesting Access to Your Health Care Records

You have the right to request access to your health care records maintained by this practice.

To request a copy of your records:

• Submit a written request through the secure client portal
• Or email: hello@innersolacetherapy.onmicrosoft.com

Please include your full name, date of birth, and dates of service (if known) in your request.

Records are maintained in accordance with Texas law. Adult client records are retained for a minimum of 7 years from the last date of service. Records for minor clients are retained for at least 5 years after the minor reaches age 18, unless otherwise required by law.

Administrative fees permitted by state and federal law may apply. You will be notified of any applicable fees prior to processing your request.
