Below you will find information regarding your rights as a client, including how to request records and how to contact regulatory or consumer agencies if needed.

Requesting Access to Your Health Care Records

You have the right to request access to your health care records maintained by this practice.

To request a copy of your records:

Please include your full name, date of birth, and dates of service (if known) in your request.

Records are maintained in accordance with Texas law. Adult client records are retained for a minimum of 7 years from the last date of service. Records for minor clients are retained for at least 5 years after the minor reaches age 18, unless otherwise required by law.

Administrative fees permitted by state and federal law may apply. You will be notified of any applicable fees prior to processing your request.

Filing a Consumer Complaint

If you believe you have been harmed by fraudulent, deceptive, or unlawful business practices, you may file a consumer complaint with the:
